What is the Patient Portal?

The Patient Portal is a secure, confidential and easy-to-use website that provides you as a patient 24-hour access to your health information from any device with an internet connection. To access the Patient Portal, you must have a username and password.

On the Patient Portal, you can:

View test and lab results (diagnostic tests)
Request medication refills
Send and receive messages securely
Access your family’s health information all in one app
Schedule appointments online
Access clinical reports – review your doctor’s notes

By accessing our Patient Portal, you agree to the following terms and conditions:

I release Salud Integral en la Montaña, Inc. from any legal responsibility that may arise from the disclosure of information and waive my rights to the confidentiality of the documents provided through the portal.

If the patient is not the applicant, a written authorization signed by the patient must be provided. Otherwise, the request will be denied.

If the patient is a minor, their guardian or legal tutor must visit the Patient Services Department with written consent. In the absence of such consent, an original copy of a court order must be provided for the portal request to be accepted.

After accepting the terms and conditions, the system will synchronize your clinical information and redirect you to the Portal, where your information will be readily accessible.
