Salud en el Hogar La Montaña program is designed for patients with physical or neurological limitations that prevent them from receiving outpatient healthcare. It includes temporary rehabilitation services under the direction of the patient’s primary care physician.

Eligibility requirements
- Have a medical condition that cannot be treated on an outpatient basis and requires the services of nursing, physical therapy, among others.
