The Case Manager conducts a comprehensive and collaborative evaluation to provide planning, facilitation, and advocacy for options and services that meet individual physical and behavioral health needs.
We offer this service at our Heath Center in Naranjito Monday through Friday from 8:00 a.m. to 4:30 p.m.
